For NDIS support coordinators

Refer a participant for positive behaviour support.

Our guided referral form collects the participant, funding and contact details needed for intake. It usually takes 7–10 minutes, you can go back at any time, and documents are helpful but optional.

Before you start

What we need from you.

You’ll need the participant’s contact details and a short description of the support needed. Fields marked Required must be completed.

  • Your details, so we know who made the referral
  • The participant’s details: the person who may receive support
  • At least one reliable way to get in touch with the participant or their nominated contact
  • What support would help and, if you can, what is happening at the moment and what a good outcome would look like
  • The funding type, if known: agency-managed, plan-managed or self-managed
  • Optional: the Behaviour Support section of the NDIS plan and any reports that will help intake review the referral (PDF, JPG or PNG, 5 MB each)

How much funding is usually needed?

We typically need at least 15–20 hours of funding for an assessment and an initial behaviour support plan. If less is available, we can often start with a letter or a smaller piece of work.

We’re not an assessment-only service. Where the plan has more behaviour support funding, we continue with implementation and training for the participant, family and support team, then monitoring and review of the plan.
